Ascended Weapons
To change the stats of an Ascended Weapon you will need 4 items
Exotic Inscription With The Desired Stats (I.E Berserker's Orichalcum Imbued Inscription)
5 Globs of Ectoplasm
Ascended Weapons
You can get Ascended weapons from different sources, such as Fractals of the Mist, Crafting, or even as a random Weapon Box drop in World vs. World. The boxes will let you choose one weapon type with the predetermined stats from the box type.
Weapon Variants - Changing Stats Will Also Change The Appearance
Exotic Inscriptions
To get the Exotic Inscription, you can simply craft this with your stats you would like, you will need a level of 400 in one of the weapon crafting professions - Artificer, Weaponsmith, or Huntsman - or simply buy one from the Black Lion Trading Post.
Globs of Ectoplasm
You can get the Ectoplasm from salvaging Rare/Exotic gear, or buy these from the Black Lion Trading Post.
Anthology of Heroes
The Anthology of Heroes can only be acquired by buying it with Spirit Shards from a Mystic Forge Attendant
Once you have these four items simply place them into the Mystic Forge and it will give you the weapon back with the Inscription's Stats, and a new skin to match those stats!
*Make sure to remove any infusions/upgrades you may have in the weapon as you will lose them otherwise*
Ascended Armor
Ascended Armor works the same way as the weapons for stat changing except you will need an insignia
Exotic Insignia With The Desired Stats

Getting the gear and such is the same process as above, Ascended armor can be acquired through the same means, Fractals, Crafting, random box drops, some can be got from achievements as well.
Exotic Insignia
To get the Exotic Insignias you can simply craft this with your stats you would like, you will need a level of 400 in one of the armor crafting professions - Armorsmith, Tailoring, or Leatherworker - or simply buy one from the Black Lion Trading Post.
The rest will be the same as the weapons - simply toss these four items into the Mystic Forge and it will give you the new item with the selected stats!
*Make sure to remove any infusions/upgrades you may have in the weapon as you will lose them otherwise*
